Difficulties
Beating the final boss on The Cinder Crown 7 opens the next difficulty. Nightmare and Inferno replay the same zones, but every enemy is level 70, health and damage are multiplied, and loot, gold and item level grow with the level the map had on Normal. That last part is why late maps are worth more on the harder difficulties even though the enemies are the same level everywhere.

What each difficulty unlocks
- Black Mist legendaries can drop from Nightmare on, at 0.3% of legendary drops from level 70 enemies. They arrive unrevealed at item level 850 and let you pick each affix from two choices.
- Divine items and set runes only drop on Inferno. Divine items have fixed stats and item level 990.
- Mythic Rift needs Inferno.
- Ancient legendaries (every affix at its maximum roll) drop on any difficulty, 10% of legendary drops from level 70 enemies.
